As of Nov. 4, Erie Evanston/Skokie Health Center opened its doors at 1285 Hartrey Ave. (at the corner of Dempster Street. Since opening earlier this year in its former location in the Morton Civic Center, the center has served 1,400 medical and 850 dental patients.

The new health center will provide pediatrics, internal medicine, family medicine, women’s health, oral health services for children and behavioral health care.

Dental Care for Children:  In the new location, there are five dental chairs and the capacity to serve new pediatric patients. Erie’s dental staff treats oral health problems, provides dental sealants and invites patients back every three to six months for cleanings, x-rays and other preventive services. No waiting list or referrals are needed for children under age 20.

Prenatal Care: Erie will offer prenatal care as well as verification of pregnancy services during our regular hours of operation. These services will include counseling and benefits assistance.

Health Education: The new location will offer onsite community education classes in nutrition, physical activity and other health topics. These classes have been well-received and have proven successful in helping patients manage and improve their health.

Expanded Behavioral Health: Erie views behavioral health care as an essential component of the medical home and will provide more than 800 patient visits per year at the new location.

Family-Centered Environment: The new site has a dedicated children’s playroom, creating a welcome distraction from the potential fear associated with visiting the doctor. For new mothers, there will be a room dedicated to breastfeeding or pumping.

Case Management: Case Managers at the new location will work with pregnant women and teen mothers and their children to link them to public benefits as appropriate and connect them with other community resources such as housing, education and subsidized cribs and car seats.

In addition to building referral relationships, Erie has also been participating in a variety of community events such as Hispanic Heritage Day, the Evanston Community Picnic, ward meetings and other activities to spread the word about the new location. Patients can call 847-666-3494 to make an appointment.
