COVID-19 Vaccinations Available for Evanston Students 12 and Older on May 21 and 22

Community members 12 and older can register to receive the Pfizer COVID-19 vaccine at two upcoming events in Evanston.

Last week, the U.S. Centers for Disease Control and Prevention authorized the Pfizer vaccine as safe and effective for individuals 12 and older. The City of Evanston’s Health & Human Services Department strongly recommends that all eligible individuals get the COVID-19 vaccine as soon as possible. 

City of Evanston Vaccination Event on May 21

The City of Evanston will host a COVID-19 vaccination clinic at Blomquist Recreation Center, 617 Foster St., from 7:30 a.m. to 3:30 p.m. on May 21.  Registration is now available at cityofevanston.org/vaccine.

The event is open to anyone 12 and older who lives, works, or goes to school in Evanston. Individuals under 18 must be accompanied by their parent or guardian or must bring a signed vaccine consent form.

Vaccines will be administered free of charge. Health insurance is not required and will not be requested. Individuals should bring a driver’s license, photo ID, or other documentation showing that they live, work, or go to school in Evanston.

ETHS/District 65 Vaccination Event on May 22

Evanston Township High School and School District 65 have partnered with Walgreens to host a COVID-19 vaccination event at the ETHS Fieldhouse, 1600 Dodge Ave., from 8 a.m. to 4 p.m. on  May 22. Registration is now available at jotform.com/form/211334240722038.

Any Evanston public or private school student (including Skokie residents attending District 65) 12 to 18 or adult members of their family may register for the event if they have not yet received the COVID-19 vaccine. This event is also open to school staff who have not been vaccinated. Individuals under the age of 18 must be accompanied by a parent or legal guardian.

Individuals should print double-sided and complete the patient consent form in advance (English/Spanish), and bring a printed copy of their ID and insurance card on a single sheet of paper. There is no charge for the vaccine, and it is available to those without insurance and regardless of immigration status. More information may be found at eths.k12.il.us and district65.net.

Vacunas Contra el COVID-19 Disponibles para Estudiantes de Evanston de 12 Años y Mayores el 21 y 22 de Mayo

Miembros de la comunidad de 12 años o más ya pueden registrarse para recibir la vacuna Pfizer contra el COVID-19 en en las próximas clínicas móviles en Evanston que se llevarán a cabo el 21 y 22 de mayo.

La semana pasada, los Centros para el Control y la Prevención de Enfermedades (CDC por sus siglas en inglés) de EE.UU. Autorizaron la vacuna Pfizer como segura y eficaz para personas mayores de 12 años. El Departamento de Salud y Servicios Humanos de la Ciudad de Evanston recomienda encarecidamente que todas las personas elegibles reciban la vacuna contra el COVID-19 lo antes posible.

Evento de Vacunación de la Ciudad de Evanston, 21 de Mayo

La Ciudad de Evanston organizará una clínica móvil de vacunación contra el COVID-19 en el Centro Recreativo Blomquist, 617 Foster St., el viernes 21 de mayo de 7:30 a.m. a 3:30 p.m. La inscripción ya está disponible en cityofevanston.org/vaccine.

El evento está abierto a cualquier persona mayor de 12 años que viva, trabaje o asista a la escuela en Evanston. Las personas menores de 18 años deben estar acompañadas por sus padres o tutores legales, o deben traer un formulario de consentimiento de vacuna firmado.
