The Evanston City Clerk’s Office, located at 2100 Ridge Ave., Room 1200, now offers vital records, including birth and death certificates, to eligible individuals upon request. Those who wish to obtain certain vital records must submit one or more valid, readable and current government-issued identifications along with the appropriate form below:
The Evanston City Clerk’s Office keeps official records of births that occur in the City of Evanston.
Under state law, an individual may legally obtain a certified copy of their own birth certificate if they are of legal age and/or the birth certificate of their child (their name must appear on the child’s birth certificate). Court-appointed legal guardians may obtain birth certificates of the children under their care if they submit a certified copy of the appointment.
Birth certificates are $15 and additional copies of the same records are $4 each.
Under state law, an individual may legally obtain a copy of a death certificate of a relative or if they can prove a financial interest in an individual’s death.
Death certificates are $17 and additional copies of the same record are $6 each.
How to request a vital record
In Person: Birth/death records can be picked up in person beginning Friday, October 29.
Online: Beginning Friday, October 29, records may be requested online.
By Mail: Required materials can be mailed to the Evanston City Clerk’s Office at 2100 Ridge Avenue, Evanston, IL 60201. On average, mail orders are processed within five working days from the date the request is received.
Vital records filed between November 2018 and October 17, 2021, must be obtained through the Cook County Clerk’s Office.
